Stockport HGV Training Centre Ltd - Checklist for Category C for Holders of Category B Licences

Steps	ü	Notes
EITHER a Full Category B Paper Driving Licence.
AND Passport or Birth Certificate.
AND Signed Notary Form from a Professional Person of 2 years acquaintance.
AND Two passport photographs taken against a plain background.
OR Full Category B Photo Driving Licence.
Obtain the HGV Provisional Licence Application Form (D1).		Forms are available from Stockport HGV as part of our Pre-Tuition Package, or your local Post Office.
Obtain the Medical Report Form (D4).		Forms are available from Stockport HGV as part of our Pre-Tuition Package, or your local Post Office.
Arrange a medical appointment with your doctor and take form D4 with you. Costs vary for medical appointments. Ask your GP.		An appointment can be made with our doctor at a time to suit you. The cost of the medical appointment is included as part of our Pre-Tuition Package.
Complete and sign form D1.		Completion of these forms is included as part of our Pre-Tuition Package.
Post your Class B licence, proofs of ID, forms D1, D4 and D750 and a cheque to the DVLA in Swansea.		The cost of the licence application is included as part of our Pre-Tuition Package.
Obtain the book "The Official Theory Test for drivers of Large Vehicles". Learn this book whilst you wait for your licence to be returned from the DVLA in Swansea. Proper home study of this book is essential.		The book "The Official Theory Test for drivers of Large Vehicles" is available from Stockport HGV, and is included as part of our Pre-Tuition Package.

On receiving your Provisional Licence, book your Theory Test either by calling 0870 0101 372 or online by clicking the DSA logo on the left.		The cost of the Theory Test is included as part of our Pre-Tuition Package and we will also be happy to book this test on your behalf, at a time and place to suit you.
Obtain your Theory Test Pass Certificate.
Take your 2 Hour Assessment Lesson with Stockport HGV. This can be done on the same day as your Theory Test.		The cost of the 2 Hour Assessment Lesson is included as part of our Pre-Tuition Package.
After your 2 Hour Assessment Lesson you will be informed of how many hours tuition you require.
Book your course and test date with Stockport HGV.		A monetary deposit is required at this point.
Obtain the book "The Highway Code". You can get this book directly from Stockport HGV, or online from Amazon. Learn this book before and during your tuition with Stockport HGV.		It is also recommended that you read the following books... "Driving Goods Vehicles: The Official DSA Syllabus" "Know Your Traffic Signals" "Driving: The Essential Skills"
Start your tuition.		The balance of the tuition payment must be paid before the course starts.
On test day bring the following documents with you. Provisional licence Theory Test pass certificate Photo licence Paper licence and passport		These documents will be presented to the examiner at the end of your test.
On passing your test, post the following items to the DVLA in Swansea. Pass Certificate Provisional Licence		A full Class C licence will be returned to you in approximately 2 weeks.
